Son of Mr. and Mrs. Walter Monk, dentist, of 1b, Bolton Road, Moses Gate, Farnworth, Lancashire. Died of pneumonia at Queen Alexandra Hospital, London and interred 24/02/1919 at Bolton (Heaton) Cemetery. Recorded as Herbert in F&WJ.
