Discharged 26/01/1917 suffering from Gas Poisoning. Lately resided at 108, Peel Street, Farnworth, Lancashire. Next of kin listed as Susan Wilkinson, of 12, Duke Street, St. Helens, Lancashire. Died from consumption following two operations of the effects of gas. Interred 25/03/1918 in Farnworth Cemetery. Recorded as W N Wilkinson in F&WJ.
