Son of Richard and Mary A. Chadwick, of 9, Foundry Square, Bolton, Lancashire. Formerly worked as a vendor for the Bolton Evening News. Enlisted June 1915. Previously enlisted aged 16 in September 1914 into the Royal Army Medical Corps, and discharged after 9 months training on account of his age. Re-enlisted and joined the Welsh Regiment. Reported missing since 10/07/1916 and subsequently presumed killed.
