Son of George Boothby Slack and Mary Elizabeth Slack, of “Calder Bank”, Davenport Crescent, Stockport, Cheshire. Enlisted February 1917. Died of pneumonia at 58th Casualty Clearing Station at Lillers. His brother George also served in the Loyal North Lancashire Regiment as Lieutenant.
